To take advantage of this offer you will need to have had your flu shot between 11/7 – 11/30, then mail in the original receipt, showing the price paid for the flu shot and include the completed form you can find HERE. There is a limit of one rebate per household and this offer is valid in the US (except RI). Your rebate form must be postmarked by 12/30 to qualify and no P.O. Boxes will be accepted.
